Als Sinterklaas bij het examen voor zijn 'dakbewijs' een ongeluk krijgt, moet hij naar het ziekenhuis. Iedereen vindt het heel erg, behalve de gemene Dr. Brein. Zij wil deze kans gebruiken om zelf Sinterklaas te worden en zo het feest voor altijd te veranderen.
