Als Sinterklaas bij het examen voor zijn 'dakbewijs' een ongeluk krijgt, moet hij naar het ziekenhuis. Iedereen vindt het heel erg, behalve de gemene Dr. Brein. Zij wil deze kans gebruiken om zelf Sinterklaas te worden en zo het feest voor altijd te veranderen. Dr. Brein probeert met haar handlangers Joris en Boris het geld voor de cadeautjes in te pikken en er met het Grote Boek van Sinterklaas vandoor te gaan. Als Agent de Bok zijn baan kwijtraakt gaat hij samen met de Pieten op onderzoek uit.
